Public Records Requests

For all public records requests, we ask that you are as specific as possible in your request for any documents.

Fees associated with Public Records Requests:

• $25 non-refundable programming fee
• $15 category fee (depends on how many licensee categories you choose)
• $0.035 per licensee provided

Next Steps:

Once you have filled out and submitted the below Public Records Request Form, the Barbering & Cosmetology Board will confirm receipt within five business days persuant to House Bill 2587. The Board will then determine the amount of records and follow up with an emailed invoice. Once the invoice is paid via money order, the records will be released.

License Verification:

If you are moving to another state and need a license certification, please fill out this form. The certificate will state your name, type of license, license number, issue date, date of expiration, disciplinary action or lack thereof, basis of licensure, and the licensure requirements.
