Salon Applications

Please do not submit a salon application if you are renewing your salon license. Go to the homepage select 'License Renewal'

Please read instructions carefully prior to filling out the application. Application must be complete to process. 

***Please note that not all phones are compatible so we recommend you use a desktop/laptop computer to apply.


STEP ONE:  When you're ready to proceed, you will click on "Apply Now" below and you'll be taken to the credit card payment screen. Key in the required information and click Pay Now. You will pay the $110.00 application fee per license and a $3.00 convienence fee. Applications take 5 to 8 weeks to process.

Note: If you are applying for two types of licenses-example Nail and Aesthetics, you must pay $110 per license-which means you need to make two payments separately (to do that you must go through the payment process twice) and submit only one application-indicate by selecting both types when you fill out the application.

STEP TWO:  If you are the Sole Salon Applicant, you will receive a link to "Complete the Salon Application".  The online application form will open in Adobe Sign and allow for online submission. Please note that if you close the application, you will not be able to re-enter. Please ensure you have all of the required documentation when starting the application. If you close out of the application, you will need to contact the Call Center and restart your application. 

If you are applying with Co-Applicants, a Corporation or LLC or a Salon within a Salon you will receive the application within 1-2 business days.  You must add all the information on the payment screen to ensure all applicants are able to sign the document.

Pursuant to A.A.C. R4-10-106(D), the Board will consider an application withdrawn if within 180 days from the application submission date the applicant fails to supply the missing information under A.A.C. R4-10-106(B)(1).  

 

Additional Information

Only completed applications will be processed; a complete application includes all applicable supporting documents and fees. Failure to complete the
application may result in withdrawal of application.
1. A change in salon name, ownership or location requires the submission of a new application with the appropriate fee ($110). FEES ARE NON-REFUNDABLE.
2. Salons must conform to all governmental requirements including zoning.
3. Ownership requires submittal of Federal Tax Identification number.

Owners must register salon name with the Secretary of State prior to submitting an application. If purchasing a salon and trade name from prior owner, please list this information on the application.
